The book came out of building the thing it describes, and watching it fail in the specific way that gave the book its title.
Tom H. Olakitan is the founder of Cerebrium Consulting and the creator of Compliance Charter, an AI governance platform for regulated healthcare professionals. A Fellow of the Association of Chartered Certified Accountants (FCCA), a Chartered Accountant of Nigeria (ACA), and a Fellow of the Canadian Association of Management Consultants (FCMC), he holds an MBA in Strategy and Digital Transformation. Eighteen years across finance, strategy, consulting, banking, and manufacturing on two continents, turning business intent into what actually gets built.
Why this book exists
Compliance Charter was built for physicians and regulated health professionals in British Columbia, where an answer that is merely plausible is not good enough and the consequence of a wrong one lands on a person's licence.
The demo went well. Production went differently. Not dramatically: no outage, no obvious hallucination, nothing that would appear on a dashboard. What appeared instead were outputs that were confident, reasonable, and that nobody had actually understood, checked, or agreed to own.
That is a dark decision, and the fix was never a better model. It was an architecture, and the book is that architecture written down so somebody else does not have to spend eighteen months discovering it.
Cerebrium Consulting
Cerebrium is an AI transformation and business advisory firm in British Columbia, working with small and mid-sized businesses of roughly ten to two hundred people, in Canada and in West Africa.
The work is upstream of tool selection: readiness assessment, data foundation, change architecture, and governance, in that order, because most failed AI initiatives were decided before anyone chose a vendor. The through-line is the same one the book argues: clarity, then control, then growth, because you cannot scale what you do not control.
